Chart Summarization

SOAP Note

Problem List

Differential Diagnosis

Clinical Decision Support

Point-of-Care Evidence
AI or database tools that retrieve clinical literature and guidance during a visit.

Never-skilling
Never learning a foundational skill because AI was available from day one.
Productive Friction
Useful struggle that forces thinking before the tool supplies an answer.
Automation Bias
Over-trusting a computer suggestion and under-checking it yourself.
Ungrounded Output
AI text that is fluent but not supported by the patient's chart or cited evidence.
Alert Fatigue
Ignoring warnings because too many low-value alerts fire every day.
Note Attestation
The clinician's sign-off that a note is accurate and ready to enter the legal record.
Patient Disclosure
Telling the patient when AI tools are listening, drafting, or informing care.
EHR
Electronic health record. The shared digital chart used across the care team.
CPOE
Computerized provider order entry. Entering meds, labs, and imaging orders in the EHR.
Order Set
A reusable bundle of related orders for a common condition or pathway.
Inbox Burden
The pile of messages, results, and refill requests that pull clinicians into after-hours EHR work.
FHIR
Fast Healthcare Interoperability Resources. A common standard for exchanging clinical data between systems.
SaMD
Software as a Medical Device. Software that meets regulatory criteria as a medical device, including some CDS.
Change Management
The people and process work that helps a new tool stick: training, champions, feedback, and rollout pacing.
Model Stewardship
Owning how a clinical AI model is monitored, updated, and retired after launch.
Unaided Competency
